Insurance
Insurance is a benefit that you work hard for, and we are committed to helping you maximize your benefits. Because policies can and will vary, we can only estimate the amount of your coverage in good faith. Insurance contracts can be very complex, and we cannot guarantee coverage. We strive to keep informed and understand insurance issues and assist you in being a good consumer. Our fees generally, but not necessarily fall within the usual and customary fee structure. Upon request, a predetermination of benefits can be provided. Should your insurance coverage change during the course of treatment, the responsible party on the account would be liable for all charges.
Payment Options 
We accept payment in the form of cash, checks, Discover/Visa/MasterCard and debit cards that do not require a pin entry. We request that the uninsured balance be paid at the time of service. We do offer payment plan options are provided through CareCredit, a healthcare line of credit.
A 5% discount is offered when full payment is made at the time of service. We also offer a 10% discount to our patients over the age of 65, but this discount does require full payment at the time of service with cash or check.
